The patient will be contacted via telephone with the date and time of the appointment, and your office will receive a fax with this information. A thorough informational packet will also be mailed to the patient regarding the upcoming appointment.
It is essential that this form is completed with the most up-to-date patient information, including the patient’s chief complaint, clinical notes related to the proble and prior 3 months’ notes, current medications and the type of evaluation requested.
