This page provides all the information and forms necessary to submit evaluation requests to Maine Medical Partners Neurosurgery & Spine. Our Intake Team is here to assist you, answer any questions and ensure that your referrals are handled expeditiously.

YOUR Patient Registration TEAM and the Intake Process:

To request a consult for one of your patients at Maine Medical Partners Neurosurgery & Spine – the Request for Evaluation form and requested materials should be faxed to (207) 885-4406. Telephone inquiries should be directed to (207) 885-4482. Upon receipt of the completed Request for Evaluation form and copies of the most recent clinical notes and diagnostic reports, our physicians will determine the urgency of the patient’s problem and make an appointment for the patient. Please note that we cannot process any intake without all of the requested information.

The patient will be contacted via telephone with the date and time of the appointment, and your office will receive a fax with this information. A thorough informational packet will also be mailed to the patient regarding the upcoming appointment.

The Form – Request for Evaluation

It is essential that this form is completed with the most up-to-date patient information, including the patient’s chief complaint, clinical notes related to the proble and prior 3 months’ notes, current medications and the type of evaluation requested. This form should be faxed to (207) 885-4406, for both neurosurgical and spine center referrals. Remember – the more detailed information we receive, the quicker we are able to determine and address the specific needs of the patient.
